The Supplement to ACORD® 25[1],[2] (A25 01G (11-16)) and Supplement to ACORD® 25 (Construction)[1],[2] (A25 01C (11-16)) have been reviewed and approved in the listed states.
If a listed state is not appropriate and your state does not prohibit use of the Supplement to ACORD® 25 forms, use the form(s) found under the heading ‘Other States (where not strictly prohibited)’ and/or ‘All States (where not strictly prohibited).’

LOUISIANA
Effective August 1, 2016, the state of Louisiana made changes to La. R.S. 22:890 by enacting Act 278. This change withdrew the approval of all certificate forms except those promulgated by an insurer, ACORD, AAIS or ISO. As a result we can no longer accept our Louisiana proprietary forms but will need policy forms or endorsements or other insurer generated evidence of coverage for requirements not evidenced on an authorized certificate form. Such forms or endorsements should clearly highlight or reference the applicable language.
